The forms are published as pdf documents. You should download, save, upload and print them. Complete the form clearly using a black pen. Incorrectly completed or illegible forms will be returned to the student. Including the sheet that you are to type about your mentor (no more than 9 lines), there are 5 pages.

A Handbook for Students and Their Parents includes information about the summer experience upon which you are about to embark. It should answer questions about the campus of West Liberty University, dorm facilities, what to pack, what to leave at home, rules and regulations.
